Here is step by step method of taking measurement.

UPPER BUST

Wrap the measuring tape around your body, passing it exactly above your bust and close to your underarm, ensuring the tape is straight and level.

Wrap the measuring tape around your body, ensuring it passes over the fullest part of your bust, right at the centerline, without going over the top or under the breast.

UNDER BUST

Mark the spot where your blouse will end and measure your body circumference at that point. Wrap the measuring tape around your chest at this level, also known as the upper waist.

SHOULDER WIDTH

Measure from the back of your neck, across the top of your shoulders, from one edge to the other. Keep your body straight and do not bend or hunch your shoulders while taking the measurement.

Measure the circumference around the shoulder edge, with the tape passing through the underarm at that level.

Measure the circumference of your arm at the level where you want your sleeves to end (cap, short, elbow length, 3/4 length, or long).

Measure from the middle of the top of your shoulder to the point where you want your blouse or choli to end. This is typically about 1-3 inches below the lower edge of your bust, usually around 14-15 inches.

Measure from the middle of the shoulder top (or where the neck width ends) to the nipple of the breast.

Neck Front 

This measurement refers to the depth of the neckline on the front of your blouse or choli. Measure from the middle of the top of your shoulder (or where the neck width ends) to the center point on the front of your chest, diagonally. The depth usually ranges from 7" to 9". The design of the blouse will largely determine this measurement (e.g., high neck, boat neck, Chinese collar may require different measurements).

Neck Back

This measurement refers to the depth of the neckline on the back of your blouse or choli. Measure from the middle of the top of your shoulder (or where the neck width ends) to the center point on the back of your chest, diagonally. The depth usually ranges from 7" to 12". This measurement is largely defined by the design of the blouse. For high necks, boat necks, chinese collars, and similar designs, you will need to measure the neck circumference. For deep necks, backless designs, sheer backs, window blouses, and those with string necklines, this measurement will differ.
(Note: This measurement is not needed for custom blouses that are already stitched, as alterations are usually not possible.)
